These are some good finds, especially the Resident Evil one; I can contribute a bit here myself.
Juniper Lee I think I know which episode you are talking about, and I believe it is either the second or third episode you're looking for. There may be more, but I'm not sure.
Kim Possible: Kim loses a shoe after being attacked by a giant poodle... and Rufus gets to ride in it!.
Teen Titans: The episode where Beast Boy turns into a dog and gets kidnapped by a rock monster/alien becuase he's mistaken for a green alien dog. In one scene the green dog runs off with Starfire's boot in its mouth, with Starfire chasing after it hopping on one foot, another with Starfire finding it all slobbery on the ground and having to put it back on.
Scooby-Doo: 3 loss here, all by the gorgeous Daphne Blake. The most well known would be this one from The 13 Ghosts of Scooby-Doo, which Mandrake posted earlier. One other I saw from What's New Scooby-Doo, where the gang have to get in the van when the parking lot is being flooded, and Daphne gets yanked in by Scooby, losing a shoe in the process, also posted by Mandrake. The last one I actually saw on the DVD of the live-action Scooby-Doo movie. There was originally going to be an animated opening scene that leads to the live-action bit, showing of the gang. Once it got to Daphne, a hand sprouted out of nowhere, grabbed her by the head, and yanked her upward, with her losing a shoe in the process.
Read Or Die: The TV: Practically every episode focuses on someone's feet. I only got about halfway through the series (its only 26 episodes), but I saw 3 losses, once by Anita, and twice by Maggie.
Maburaho: Episode 21 mimics Cinderella with Kuriko losing her shoe on the steps.
Hayate the Combat Butler: In Episode 28, Shion is defeated by Hayate after he knocks her boots off (17:55). She loses one again in Episode 34 (6:12).
The Cat's Return: This one happens within the first 2 minutes! the main character is running and she stumbles and loses her shoe. Nice shot and great animation.
Tekken: Here's one good scene from the original Tekken. While it's not a loss, it's still amusing.
inFAMOUS: Festival of Blood. A vamp'd Cole MacGrath bites a woman and she loses a shoe; we don't get to see the loss, but we see both her shoe and her bare foot.
One Missed Call: Near the end of the original Japanese version, the main character gets dragged down the hallway ands stopps halfway through a door, and her cowboy boot pops right off her foot before she crawls away. It might be in the American version, but I'm not sure.
Power Rangers: Lightspeed Rescue: In the episode "Web War", Ms. Fairweather loses her shoe after being attacked my the main villain Arachnor.
...this is all off the top of my head; I'll have to do some research to find some more goodies for ya.
